Exam booking form

This form should be completed fully and returned to the NCTJ to arrive at least TWO WEEKS before the exam date. Separate forms should be completed for each exam date required.

Late applications may be accepted up to two days after the deadline however an administration fee of £10 + vat will apply.

All fields are required. If you work on a purchase order system the purchase order number must be inserted in the Place of Exam box after you have typed in the address.

Before commencing this exam, please read the rules and regulations. These must be adhered to and the "Procedure for Candidates" must be handed out. Any departure from the NCTJ's conditions will render the examination void.
